Issue: On certain vehicles converted for wheelchair access by Vantage Mobility, the wheelchair tie-down retractors may not have been manufactured properly. As a result, the retractor(s) may not lock automatically. If this happens, the wheelchair may not be secured correctly and could move in a crash. Note: This recall only affects certain Q'Straint QRT-DELUXE and QRT-MAX wheelchair retractors with the part number Q8-6200 or Q8-6209. Safety Risk: A wheelchair that moves in a crash could increase the risk of injury. Corrective Actions: To reduce the safety risk, Vantage Mobility recommends you confirm the wheelchair is secured properly by attempting to move it before driving until the recall repairs have been completed. Vantage Mobility will notify owners by mail and advise you to take your vehicle to a Vantage Mobility service center to inspect and, if necessary, replace the wheelchair retractor(s).
Issue: On a small number of vehicles, the bolt that secures the third-row center seat belt anchor to the roof may not have been installed properly. As a result, the seat belt could loosen or detach in a crash. Safety Risk: A seat belt that detaches in a crash could increase the risk of injury. Corrective Actions: To reduce the safety risk, Toyota advises that you shouldn't transport a passenger in the third-row center seat until the recall repairs are completed. Toyota will notify owners by mail and advise you to take your vehicle to a dealership to replace the third-row center seat belt upper anchor bolt.
Issue: On certain vehicles, a bolt in the inverter assembly for the hybrid system may not have been tightened properly. As a result, the vehicle could enter a reduced power mode or lose power to the wheels. Additionally, if the bolt becomes loose in the inverter assembly and the ignition is on, it could cause a short circuit. Safety Risk: A loss of power to the wheels could increase the risk of a crash. A short circuit can create the risk of a fire. Corrective Actions: Toyota will notify owners by mail. The corrective actions for this recall are under development.

As a result, the retractor(s) may not lock automatically. If this happens, the wheelchair may not be secured correctly and could move in a crash. Issue: On certain vehicles, the engine may not have been manufactured properly. As a result, metal debris could cause the crankshaft bearings to fail. If this happens, the engine may run rough or may not start. This could also cause the engine to stall while driving. Note: This recall only affects certain vehicles equipped with a 3.4L twin-turbo engine. This recall is an expansion of Transport Canada Recall No. 2024-304. Safety Risk: A sudden loss of power to the wheels could increase the risk of a crash. Corrective Actions: The company will notify owners by mail. The corrective actions for this recall are under development.
Issue: Under certain conditions, a software problem could cause the rearview camera image to freeze or not to display. If this happens, the rearview image may not display when the transmission is shifted into reverse. Canadian regulations require the rearview image to display when the vehicle is reversing. Note: This recall only affects certain vehicles equipped with a panoramic view monitor system. Safety Risk: A rearview camera image that doesn't display correctly could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: The company will notify owners in writing and advise you to take your vehicle to a dealership to update the parking assist ECU software.
Issue: On certain vehicles, the second-row passenger seat rails may not have been welded properly. As a result, the seat(s) could move in a crash. Safety Risk: A seat that moves in a crash could create an increased risk of injury. Corrective Actions: To reduce the safety risk, Toyota advises that you shouldn't transport passengers in the second-row seats until the recall repairs are completed. Toyota will notify owners by mail and advise you to bring your vehicle to a dealership to replace the second-row seat rails.
Issue: Under certain conditions, a problem with the rearview camera could cause the rearview camera image not to display correctly or at all. Canadian regulations require the rearview image to display when the vehicle is reversing. Note: This recall only affects vehicles equipped with a 14-inch multimedia display. For the Tundra model, this recall affects both Tundra and Tundra hybrid models. Safety Risk: A rearview camera image that doesn't display correctly could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: Toyota will notify owners in writing and advise you to take your vehicle to a dealership to update the multimedia display software.
Issue: On certain trucks, the front axle shaft(s) may not have been manufactured properly. As a result, the axle shaft(s) could break and prevent you from fully turning the steering wheel. Additionally, if the truck is parked and the parking brake isn't applied, the truck could roll away. Safety Risk: A steering wheel that can't be turned completely could increase the risk of a crash. A parked vehicle that rolls away could create the risk of an injury or a crash. Corrective Actions: Toyota will notify owners by mail and advise you to take your truck to a dealership to inspect and, if necessary, replace the front axle shaft assemblies.
Issue: On a small number of vehicles, water could leak into the starter relay and cause it to corrode. If this happens, the relay could short circuit and the starter could overheat. Note: This recall only affects certain vehicles equipped with a 2.0 L engine. Safety Risk: A short circuit and/or a starter that overheats can create a fire risk, even while the vehicle is parked and turned off. Corrective Actions: To reduce the safety risk, Toyota advises that you should park your vehicle outdoors and away from other vehicles or structures until the recall repairs have been completed. Toyota will notify owners by mail and advise you to take your vehicle to a dealership to replace the engine starter.
