Full recall reportIssue: On certain vehicles converted for wheelchair access by Vantage Mobility, the wheelchair tie-down retractors may not have been manufactured properly. As a result, the retractor(s) may not lock automatically. If this happens, the wheelchair may not be secured correctly and could move in a crash. Note: This recall only affects certain Q'Straint QRT-DELUXE and QRT-MAX wheelchair retractors with the part number Q8-6200 or Q8-6209. Safety Risk: A wheelchair that moves in a crash could increase the risk of injury. Corrective Actions: To reduce the safety risk, Vantage Mobility recommends you confirm the wheelchair is secured properly by attempting to move it before driving until the recall repairs have been completed. Vantage Mobility will notify owners by mail and advise you to take your vehicle to a Vantage Mobility service center to inspect and, if necessary, replace the wheelchair retractor(s).

Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.
Potential consequence
An unsecured wheelchair can move during transit, increasing the risk of injury.
Remedy
Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."
Potential consequence
An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.
Remedy
Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.

Issue: Under certain conditions, the vehicle stability control (VSC) system may not switch on when the ignition is restarted. This can happen if the driver manually turns off the VSC, depresses the brake pedal, and then turns off and restarts the vehicle while continuously depressing the brake pedal. Canadian regulations require the stability control system to be active each time that you start the vehicle. Safety Risk: A stability control system that is switched off could increase the risk of a crash. Corrective Actions: The company will notify owners by mail and instruct you to take your vehicle to a dealer to update the skid control ECU software.

Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."
Potential consequence
Driving with a deactivated stability control system increases the risk of a crash.
Remedy
Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.

Potential consequence
During seat belt pre-tensioner deployment, the incorrect webbing guide may damage the seat belt webbing, causing it to tear and increasing the risk of injury during a crash.
Remedy
Dealers will replace the outboard second-row seat belt assemblies, free of charge. Owner notification letters were mailed January 10, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 21TB08 and 21TA08.
