Vantage Mobility International, LLC (Vantage) is recalling certain 2012-2018 AMS Genesis wheelchair vans, built on a Toyota Sienna vehicles. The rubber brake hoses that attach to the rear wheel assembly may be too short, causing them to rub against the rear axle trailing arm.
The contact may cause failure of the rear brake hoses, resulting in a loss of vehicle control and reduced braking ability, increasing the risk of a crash.
Vantage will notify owners, and dealers will install a set of rear brake hoses that will provide the proper clearance, free of charge. The recall began December 16, 2019. Owners may contact Vantage customer service at 1-800-488-9082.

Braun Corporation (Braun) is recalling certain 2014-2019 Toyota Sienna mobility van conversions. The brake lines may have incorrect tube nut fittings installed, which may cause a pressure drop within the braking system.
The reduction in the braking system pressure may cause a loss of braking power in the rear braking system, increasing the risk of crash.
Braun has notified owners, and dealers will install an adaptor to correct the issue, free of charge. The recall began June 25, 2019. Owners may contact Braun customer service at 1-800-488-0359.

An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.
Toyota will notify owners. Depending on the vehicle model, dealers will replace the front passenger air bag inflator, or replace the air bag assembly. The recall began February 15, 2019. If the power sliding door is unable to be opened when commanded, such as if the door is frozen shut, the door may subsequently open unexpectedly, possibly while the vehicle is moving.
If door opens while the vehicle is moving, there would be an increased risk of injury to the vehicle occupants.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2014 Sienna vehicles manufactured June 17, 2014, through June 24, 2014. The affected vehicles have a transmission shift control cable whose protective cable sleeve may have been damaged during assembly. This could cause the control cable to bind or separate during gear shift operation causing incorrect gear selection. As such, these vehicles fail to comply with the requirements of the Federal Motor Vehicle Safety Standard No. 102, "Transmission Shift Lever Sequence, Starter Interlock, and Transmission Braking Effect."
If the transmission shift cable separates while the vehicle is being driven, the transmission gear selection may not match the indicated gear and the vehicle may move in an unintended or unexpected direction, increasing the risk of a crash. Furthermore, when the driver goes to stop and park the vehicle, despite selecting the 'PARK' position, the transmission may not be in 'PARK.' If the vehicle is not in the 'PARK' position there is a risk the vehicle will roll away as the driver and other occupants exit the vehicle or anytime thereafter. A vehicle rollaway increases the risk of injury to exiting occupants and bystanders.

Issue: This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. This could cause a brake fluid leak that may result in reducing braking. Note: This problem affects conversions that were built between January 9, 2018, and May 20, 2019. Safety Risk: Reduced braking could cause extended stopping distances and increase the risk of a crash. Corrective Actions: The company will notify owners by mail and instruct them to take their vehicle to a BraunAbility dealer to inspect the brake line connections. If necessary, an adapter fitting will be installed and the brake system will be bled.

Issue: On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, may eventually degrade the propellant contained in the passenger-front airbag. This could cause the airbag to deploy with more force than normal. If the airbag inflator ruptures, fragments could be propelled toward vehicle occupants or cause damage to the airbag assembly, preventing its proper function. Safety Risk: If the vehicle is involved in a crash requiring an airbag deployment, the airbag inflator could rupture and create a risk of injury. Corrective Action: Owners will be notified by mail and instructed to take their vehicle to a dealer to have the passenger-front airbag inflator or airbag module replaced. Note: The Canadian climate results in the propellant degrading slowly. This recall is being conducted as a precaution to address future risk. It is expected that all airbag inflators will be replaced before their function would be affected.

On certain vehicles equipped with power sliding doors, under certain conditions which impede the opening of the door (such as if the door becomes frozen with ice), the sliding door motor could stall when the door is operated. If the motor stalls, high current in the door motor circuit could be generated, operating the circuit protection for the door motor. If this occurs with the sliding door latch mechanism in an unlatched position, the door could open while driving, increasing the risk of injury to a vehicle occupant. Correction: Dealers will replace the existing junction block and the rear sliding door wire harnesses with newly designed ones. The dealer will also install two sub wire harnesses to connect the junction block to the two rear door wire harnesses.
